Request Your First Appointment:

Prospective patients, please complete the appointment request form above.  Once this form is completed, I will determine if you are appropriate for my practice.  I may send you an invitation to Elation Passport (my secure patient portal) for ongoing communications.  Once we determine that we are a good fit for each other, I will advise you on the next steps (either via the portal, or over the phone if preferred).  If you have already called and left me a voicemail, please still complete this initial request.  I typically return all voicemails at the end of the day when I finish seeing patients. 

Telemedicine URL:

Please use the above URL if you have a scheduled telemedicine (online video chat) appointment scheduled with me.  The system will ask you to enter your name and “Check In” and I will join you in the private video session at your appointment time.  Depending on your computer / tablet platform, it may ask permission to access your camera.

Please utilize Elation Passport to securely communicate with me for non-emergency concerns between appointments, request appointments, and download account statements for submitting insurance reimbursement requests.  If you do not currently have an Elation Passport account, please contact my office and leave me a voicemail to send you an invitation.  This will go to your email.  Also, for iOS users, there is an app for iPhones.
