Telemedicine URL:

Please use the above URL if you have a scheduled telemedicine (online video chat) appointment scheduled with me.  The system will ask you to enter your name and “Check In” and I will join you in the private video session at your appointment time.  Depending on your computer / tablet platform, it may ask permission to access your camera.

Existing Patient Portal URL:

If you are an existing patient, you can use the portal as a method to communicate with me securely for non-emergency concerns between appointments, request appointments, see any account balances, and download account statements for submitting insurance reimbursement requests.

New / Prospective Patient URL:

My patient and provider portal provides secure and confidential access to the following:

  • Prospective patients: you can place a consultation request via my pre-screening form.  It will allow you to provide much of the information I’d need to obtain from you before or during a phone conversation to determine if we should proceed with an initial appointment.  I can obtain all of this information by phone if you prefer, but completing the form beforehand will let us discuss your treatment concerns immediately during the phone call.
  • Providers: if you are a healthcare provider and we have a mutual patient who has signed releases allowing us to collaborate on their status or treatment, we can communicate securely.  I’m of course happy to handle this communication by phone as well if that is preferred.
