** Please note that at this time, all appointments are done via telemedicine. I am not offering in-person appointments at this time. Please use the above URL if you have a scheduled telemedicine (online video chat) appointment scheduled with me. The system will ask you to enter your name and “Check In” and I will join you in the private video session at your appointment time. Depending on your computer / tablet platform, it may ask permission to access your camera.
Cancellation policy reminder and instructions: Please click on the Reschedule/Cancel to reschedule and cancel appointments. Appointments must be cancelled/rescheduled no less than 48 hours before appointment time. The full appointment cost will be automatically charged for cancellations less than 48 hours prior, as well as missed appointments. Please set reminders for your appointment. HubbardMD will attempt to send you reminders but cannot guarantee you will receive them. Appointments will be automatically charged 24 hours before appointment time. If you plan to pay by check, please notify physician at least 48 hours before the appointment. Be sure you receive confirmation from physician that she is aware of your preferred type of payment for the appointment.
**Returning patients who have booked previously using the link below: you may also hit the Reschedule/Cancel link if you wish to log-on and schedule without having to re-enter your credit card information. After you book your first appointment, your card will be kept securely on file and the system will be able to recognize you and allow you to use the card stored.

** Please note that at this time, I am not scheduling new patients. However, this form is required before you are scheduled and may be used in the future for the scheduling of patients on a potential waitlist when scheduling resumes. Closure of new patient appointments allows me to focus on my current patients and for adequate space for current patients to be seen for follow-up. ** Prospective patients, please complete the appointment request form above. Once this form is completed, I will determine if you are appropriate for my practice. I may send you an invitation to Elation Passport (my secure patient portal) for ongoing communications. Once we determine that we are a good fit for each other, I will advise you on the next steps (either via the portal, or over the phone if preferred). If you have already called and left me a voicemail, please still complete this initial request. I typically return all voicemails within 2 business days.
